This is the Tohoku University library user card issuance application form.


<Before application>
For Tohoku University members, a student ID card or faculty ID card may be issued.
These magnetic cards can be used as library user cards, so this application is not necessary. Please see the example of the library user card on the website below.

If you are Visitors and want to use only the ordering service (Photocopies / Inter Library Loan) , you do not need to apply a library user card. Please check the website below.

After applying, we will send email you as soon as your card is ready. Please come to the Engineering Library counter. (Monday-Friday, 9:00-17:00)
