Dear Parent,

Your child’s health and safety are as  important to us as it is to you. This is why it is important that you go through these questions each morning and answer them truthfully. This will help you to make a decision whether it is fine for your child to attend Small World School on that day, or whether she / he needs to stay at home.

NB NOTE: Please complete one form per child. If you have more than one child attending Small World School, and your answer to any of the below questions on their health is yes, then ALL your children should stay home.
