聲明 (Declaration)
本人欲參加中國香港賽艇協會(總稱“賽協”) 主辦、康樂及文化事務署 資助的「香港青少年賽艇學院 - 新秀招募日」，謹此聲明本人參加課程及一切有關活動(總稱“活動”)，並願意遵守由賽協所訂的條文及規則，並同意以下所列各點：
1. 本人聲明上述資料確實無誤，本人能穿著便服游泳最少 50 米距離及本人身體健康及有能力參加此活動，本人將對個人安全及財物負全責。
2. 本人同意賽協有權收集、儲存及使用本人在聲明上所填報的個人資料【個人資料(私隱)條例之定義】用作有關予該活動(包括籌備、推廣或宣傳該活動等)。本人亦同意賽協可將本人的個人資料發放予賽協及協辦機構用作有關於該活動之用。
3. 本人明白及同意提供本人的身份證或護照給予賽協(如有需要及在大會要求下)，以作核對個人資料。
4. 本人授權賽協在上述的活動中對本人進行錄像和照片拍攝，並同意賽協將來不需要徵詢本人同意下，有權使用有關照片和視頻片段進行市場推廣及教育推廣用途。
5. 本人簽署此聲明以示同意及確認所有列明之重要事項、聲明及有關細則。
As a condition of myself being permitted to compete in the above "Hong Kong Rowing Youth Academy’s Selection Day"(collectively “Event”) and in consideration of the opportunity to take part in the Event, I confirm to the Hong Kong, China Rowing Association (HKCRA), their Preparatory Committee (collectively “Organizer”) as follows,
1. I declare that the above information is correct. I can swim at least 50 meters in light clothing and physically fit and capable of participating in the course. I understand that I will take full responsibility of my safety and property.
2. I agree that the HKCRA & Organizer are permitted to collect, store and use my / my team members’ personal data (as defined in the Personal Data (Privacy)Ordinance) as provided by me in the Declaration for the purpose of or in connection with the Event (including organization, promotion, and publicity of the Event etc.). I further agree that the HKCRA may pass on my personal data to such of its supporting organizations for the purpose of or in connection with the Event.
3. I understand and agree to provide my HKID Card/Passport to verify my/ my team members’ personal details to the HKCRA upon request.
4. I also grant the right to HKCRA to film video and take photographs of me / my child in the above-mentioned audition. I understand that The Hong Kong China Rowing Association will use photos and video clips from the above-mentioned audition for marketing, promotional and educational purpose.
5.By signing this Declaration, I agree to and confirm to accept all of the terms, conditions and points made and subsequently raised in the important notes, declarations and conditions.
