 1. Physical and Physical Form Turned into office (Davies or Discovery)
 2. HIPPA and Concussion Forms (On parent PowerSchool)
 3. Pay activity fee

-First 3 days of practice will be team's "try-out days." Davies softball has a JV and Varsity team, cuts may be made depending on number of people who try-out. Middle schoolers ARE allowed to try-out for the high-school teams!
