Calvary Hill Church Service Registration - Sunday, December 13

We are SO excited to be able to gather to some extent and worship together on Sundays! Please help us keep attendance to no more than 100 attendees as is currently permitted by the government. After you've read through the following information, please complete, and submit this form.

IN AN EFFORT TO PROVIDE A SAFE ENVIRONMENT:
- The cafe will remain closed
- Please enter the building from the main front (cafe) doors & exit from the West doors (will be indicated)
- Allow the welcomers to open doors for you
- While families can sit together we will maintain state regulated distance between non-family members
- Services will be one hour in length
- Hand sanitizing stations will be provided
- Offering baskets will not be passed from person to person but will be held and passed by an usher
- Our "Calvary Hill HOME Church" newsletters including a video of Sunday's teaching will be provided each week for those who are not ready to attend services

PLEASE JOIN US IN OUR EFFORTS TO MAINTAIN GOVERNMENT REGULATIONS AND SAFETY IN THE FOLLOWING WAYS:
- Wear your face mask (includes children over 2 yrs of age)
- If you are bringing your children, please keep them with you at ALL times that they are not checked into Kidz Corner

While we would love to be able to provide the perfectly safe environment for everyone...
PLEASE REFRAIN FROM ATTENDING OUR SERVICES FOR ANY OF THE FOLLOWING:
- if you are immunocompromised
- if you are not feeling well or have a temperature and/or cold symptoms
- if you feel there are unanswered questions regarding the environment you will find or have any hesitations or concerns

disinfecting all surfaces in the restroom following the 9am service

disinfecting surfaces and/or doors following the 9am service

Comments:

Once you have submitted this form you should see the following immediate message displayed: "Thank you for registering! See you on SUNDAY, DEC 13th! We are looking forward to worshiping with you!" This confirms that we have received your registration. Also, a copy of your form responses will be emailed to the address you provided. You can consider this to be your registration confirmation. You will not receive additional correspondence regarding your registration unless there is pertinent information that we need to inform you of. If for any reason you are not able to attend the service, please contact us at info@calvaryhill.church so that your slots can be made available to others. Otherwise, see you Dec 13th!
