I acknowledge there is a REQUIRED Parent Meeting on 6 Mar 2023, 6:30pm, PVHS Auditorium, that I must attend with at least one parent/legal guardian to remain on the primary list to attend the Hawaii CIA Trip.   

I acknowledge there is a REQUIRED Bag Check either on Thursday, 9 March 2023, 6:00pm or Friday, 10 March 2023, 6:00pm, in the PVHS AFJROTC area.  The purpose is to weigh your one bag/suitcase that will remain locked up in the classroom until we depart on 13 March 2023 (Each Cadet gets 1 checked piece of luggage with one carry on that meets the size and weight enforced by Southwest Airlines) and check that you have ALL the required items on the packing list.

NAME FORMAT is FIRST NAME LAST NAME.  This is the name for emergencies and for our parent/guardian Remind101 communication group text.  This should also be the same person associated with the cell number your provide in the next question. 

CELL NUMBER FORMAT 123-456-7890 (Please do this correctly so we can set up our Remind 101 for the trip).  This is the cell number of the parent/guardian that will be on the Remind 101 for emergencies or general information about the trip.

Please list any medications your son or daughter must take over the duration of the summer camp.  All students are required to turn-in all medication into our designated Booster Mom for safe keeping and administering based on prescribed dosage and frequency.  Your son or daughter are responsible for coming to the Booster Mom for all medications. As a parent or guardian, I authorize my cadet to swim in the ocean.

I acknowledge my cadet must adhere to all the Rule of Engagement (ROE) on the itinerary and that any Red Lines (briefed in the Parent's Meeting) my cadet crosses will require my cadet to be sent home early from the CIA Trip at my own expense (to include all transportation, fees, meals, or any costs associated with travel home).
