Below you will find a brief form that will allow us to complete your requested order.  Please complete the form and return to us so that we can get started on your song!

There are three different options listed below. You may choose any combination of the three available packages. You only need to fill out the questions related to the package(s) you choose. Additionally you will see new upgrade opportunities to enhance your celebration package.

Choose ANY song!  Pick ANY Standard, Jazz, Broadway, or Pop song and we will perform your song choice for you along with a special video message. (PLEASE NOTE: If we are unable to perform your requested song, you will be notified of alternative similar options to choose from.)  Once the form is completed, you will receive further instructions on how to make payment.

Option Three: PLATINUM PARODY PACKAGE
PLATINUM PARODY PACKAGE - $89 Special Rate! (Regular Rate $99)  
CUSTOM PARODY song lyrics will be created and performed to the song of your choice along with a personal video message. We will use your answers from the questions below to write a custom song just for them!
PLEASE NOTE: The information you fill in below will help us to create a song parody. Just fill out whatever you can. Even tho we may not include all of the information you list below, we will do our best to incorporate as much of your information as possible.

Also, song parodies are generally fun and comedic in nature and therefore we recommend that you select a song that is up-tempo and fun to listen to.

If you prefer a love song, we recommend selecting Option 2 (the GOLD RECORD PACKAGE) and we will perform that song for you with the original lyrics as a dedication.
