A brief survey to understand mental health support after trauma
We would like to understand your experiences with mental health and accessing mental health support, especially if you have experienced trauma (you do not have to have a diagnosis).

The survey asks some questions about any symptoms you may be experiencing, the trauma and your experiences with mental health support. Please only fill in the survey if you feel physically safe and emotionally able to do so.

The survey should take approximately 5 minutes to complete.
