For value received, in my capacity as the parent or guardian of a participant, who is either a child under the age of 18 years, or an adult over the age of 18 years for whom I am legal guardian, and on behalf of such child or adult, I hereby grant to the Columbus and Central Ohio Children’s Chorus Foundation, its legal representatives, successors, and assigns, and those acting with its authority and permission (jointly and severally, “Columbus Children’s Choir”) permission for my child listed below to participate in any rehearsals, performances, travel or other opportunity. *

I agree that should my child require any medical treatment as a result of such participation or otherwise, that while Columbus Children’s Choir’s representatives’ and chaperones’ will attempt to contact me, that in the event Columbus Children’s Choir’s representatives’ and chaperones’ attempts to contact me have been unsuccessful and/or the demands of time, or the particular issue make it unreasonable or unwise or not in the child's best interest to await my contact, that any treatment deemed necessary by a treating physician, hospital or care giving facility, can be agreed to and authorized by Columbus Children’s Choir’s representatives. *
