The purpose of this survey is to submit an application for your student to attend one or more of the 2021 AMS Summer Learning Camps. The completion of this form will indicate your intent to enroll in one or more of the Summer Learning Camps.  If at a later date, you choose to withdraw from a camp, please contact the school immediately.

Your child may attend one, two, three, or four of the camps, but you must sign up for those individually here.  Camp #1 (May 24-28); Camp #2 (June 1-4) (4-Day Camp. NO school on Memorial Day); Camp #3 (June 7-11); Camp #4 (June 14-18). (You are committing to attend on these dates of each summer camp.) *
