Rural & Underserved Utah Training Experience (RUUTE) Participation & Use of Name/Likeness Release Form
The Rural & Underserved Utah Training Experience (RUUTE) program was created with the goal to increase medical education opportunities in rural and underserved areas of Utah by expanding the interest and awareness of rural health, maintaining and growing quality educational experiences, and developing and enhancing community partnerships with stakeholders. The RUUTE program has developed a K-12 outreach pipeline where the RUUTE team visits K-12 schools in rural and/or underserved areas of Utah to get students excited about the health sciences and provide them with learning experiences they may otherwise not have experienced.

This form is for student participants aged 18 years or older and the parents/guardians of underage students to fill out prior to the student's participation in any RUUTE related activities. Prior to participation in RUUTE activities, RUUTE kindly requests that student participants aged 18 years or older and the parents/guardians of minor students provide responses to the questions in this form. The data collected will be used in hopes of seeing that K-12 students who express interest in health sciences ultimately end up in that field of education and/or career. None of the information collected from this form will be shared with any parties outside of the RUUTE program and the University of Utah. The information may be used to measure the reach of our pipeline efforts and to send promotional items and/or information about the health sciences programs at the University of Utah. Elementary school students will participate in activities related to the human heart, learn about ways to lead a healthy life, and learn how to take someone's heart rate and blood pressure. There is a lecture and small hands-on learning activities.

Middle school students will participate in activities related to the human eye, the parts and functions of the eye, colorblindness, and biomedical devices and design. There is a lecture, individual and group activities, and hands-on learning.

High school students will participate in activities related to the human heart, learn about blood pressure basics, how to take someone's blood pressure, and mindfulness techniques. There is a lecture and hands-on learning through working in pairs.
