Form is to be completed within 3 days by a parent when a student is marked absent due to not attending homeroom in Microsoft Teams. The parent attests that the student did attend school on the date marked absent by selecting the method(s) of attendance listed below.
*It is recommended that parents check Power School after 3 days to verify that attendance has been corrected.
