MVHS PTSA 'Spring Into Summer' Vendor Registration Form (5/11/2019)
Registration to this year's Spring Into Summer Vendor & Community is now OPEN.

Please submit your application information below to be considered for this year's Spring event

Applications will be accepted on a first come, first serve basis.

■  DATE: Saturday, May 11, 2019

■  TIME: 10:00 AM to 3:00 PM

■  MAKE-UP/RAIN DATE: Saturday 05/25/2019
               (Back-up date in case FCPS cancels the use of the school grounds on 5/11)

■  DEADLINE TO REGISTER: Saturday, May 4th

■  EARLY BIRD REGISTRATION FEE: $40 (ends Sunday, March 31, 2019)

■  REGULAR FEE: $50 (Starts Monday, April 1st | Ends May 4th)

■  MVHS Staff & Student Vendor Fee: $30

■  Food Truck Vendor Fee: Same as Above.
     - (Must provide proof of applicable permit(s) and or licenses)

■  Vendors will primarily be indoors / Community fun activities primarily outdoors.

P.S.: Payments will be accepted via ONLINE PAYMENT after Vendor Application has been Pre-Approved. Pre-Approved vendors will receive link to make payment.

P.S.S.: We are currently NOT ACCEPTING any more applications from the following vendors:
--Tupperware
--Avon
--Paparazzi
--Color Street Nail
--Scentsy
--Jewelry (of any kind)
