In consideration for allowing my child to participate in activities at or sponsored by Oakhurst Baptist Church (OBC), I/we agree to allow a staff member or volunteers of OBC to transport my child to and from my home, or have my child transported, to the nearest medical facility in case of emergency or in the event that I cannot be reached by phone. I/we understand that there are inherent risk involved in any event, and I/we hereby release OBC, its employees, its agents, and volunteer workers from any and all liability for an injury, loss, or damage to person or property that may occur during the course of my/our child's involvement with VBS. In the event that he/she is injured while attending the Event and required the attention of a doctor, I/we consent to any reasonable medical treatment as deemed necessary by a licensed physician. I will be responsible for, and not hold OBC or individual OBC staff members or volunteers workers responsible for, any expense incurred. I give consent for my child to be photographed, videotaped while participating in activities at or sponsored by OBC and for those images to be used in publication. I understand that proper and respectful behavior is expected of my child.
