Permission of Participation Form

I give permission for my child to participate freely in all and every NOW Ministries Event at their own risk. I understand that this includes any Camps, Retreats, NOW Club events, Missions Trips, etc. I agree that NOW Ministries is NOT responsible for any harm, sickness, accident, damage, or loss with regards to the welfare of said child/children while in the care of NOW Ministries’ staff, or volunteers.
I also give my permission for the following if my child desires to participate:
1. Water Baptism
2. Spirit Baptism
3. Permission for any and all medical needs and transport to the nearest medical facility should the need arise
4. Any and all photo and video clips with permission to use child/children’s image on social media, emails and in general
