Emotional Support Animal Request and Agreement
EMU is committed to allowing individuals with disabilities the use of a service animal on campus to facilitate their full participation and equal access to EMU’s programs and activities. EMU is also committed to allowing assistance animals necessary to provide individuals with disabilities equal opportunity to use and enjoy campus housing.

This form begins the initial request to confirm eligibility for a support animal in EMU Housing. The EMU CARE Team provides assessment and support services to students seeking an Emotional Support Animal. To determine eligibility for a support animal, the CARE Team requires documentation from the licensed mental health care provider regarding consistent ongoing counseling and an established emotional support animal. The licensed provider may also attach a report providing additional related information.

To complete your request, this form must be completed confirming you have read the ESA Agreement. Following this form, you are required to submit a completed 'Provider Request for ESA Accommodation' from. Once submitted to the CARE Team, you will be notified of a decision and appropriate follow up will proceed with EMU Housing, if approved.
