Please submit this request form at least two weeks prior to the birth date, if possible.  Most birthday boxes are mailed and this will allow time to get it there before the birthday.  We will fulfill requests received up to two weeks after the birthday.

We cannot guarantee the availability of any specific item, but we will do everything we can to help make this child's birthday special!

NMFFC maintains the confidentiality of the information provided.  Eligibility for request fulfillment/awards is limited to children and youth (under the age of 18) who are currently in the custody of the State of New Mexico or a Tribe or Pueblo within New Mexico.
