Cassie Murphy-Cullen, Ph.D, Scholarship Application
This scholarship was established by former residents and colleagues of Dr. Cullen to honor her memory and to continue her legacy by providing funding to Texas family medicine residents to attend national conferences to present their research.  Both oral and poster board type presentations are acceptable. The most well-known national meetings are the NAPCRG Annual meeting, the AAFP FMX, and the STFM Annual Spring Conference – although other national meetings could qualify as well.  The scholarship is administered through the Texas Academy of Family Physicians Foundation, the philanthropic arm of the TAFP. Up to $2,200 can be awarded each year but it may be split among multiple recipients. Applicants will be notified of the decision of the review committee by mid-August.

Eligibility Criteria
1.  Applicant must be a resident in a Texas family medicine residency program and a TAFP member.
2.  Project must have been accepted for presentation at a national meeting at the time award decision is made – but you may submit application for this award once you have applied to a national meeting.
3.  Residency director must sign off that the program will not reduce any funding that would have been available to support the resident in making their presentation should the resident be awarded the scholarship.

Note:  Awards can also be given to previously presented projects – as long as the applicant is still a resident at the time the application is submitted.

Note: A combination of factors will be considered in selecting the recipient of this scholarship. Selection committee members will take into consideration the level of overall involvement of the resident in the project, from inception to completion.  Other considerations will include clarity of aims / research question and relevance of the project to family medicine.  Remembrance of Dr. Cullen's interests in patient education, behavioral health and general promotion of better health care by family medicine residents may also be considered.  We appreciate your interest in applying.

Submission instructions
In addition to this form, you must submit the following :

1. Verification that your project has been accepted for presentation at a national meeting,
2. A letter of recommendation from a faculty member familiar with your project, and
3. A note from your residency program director to attest that if you receive a scholarship it will not decrease any funding that would otherwise be available from the program to support travel to attend the meeting to present your research.
