I understand that by submitting this form I am registering for Shooting Star's 2019-2020 season. Though there is no payment penalty for withdrawing if I choose to before September I will give as much advanced notice as possible. It is also understood and agreed that Katie Hickey, trading as Shooting Star Dance Center shall not be liable for any damages arising from personal injuries sustained by the student in, on or about the premises, liable resulting from or arising out of the negligence of the agents and employees of the aforesaid dancing school, or the negligence of any other person present on said premises. I further agree to permit said Katie Hickey, trading as Shooting Star Dance Center, to use any photograph or video in which my child may appear for promotional purposes only. I also understand that tuition is due by the 8th of the month either by Auto Pay or by cash/check (due the 7th).
