SEP Science Kit Booking Form
Hello LKDSB & SCCDSB elementary Educators!  Please submit a form for EACH science kit you would like to borrow.

An email confirming your exact ship/due dates will be sent to your school board email address. 

NOTE:  Butterfly kits are not an option on this form.  They are only available to Gr. 2 teachers in April or May and in limited numbers.  Please email Wendy.Hooghiem@lkdsb.net to order a butterfly kit.
