This is the official application form for the Division of Personnel's Train the Trainer certification program. This form must be completed by the applicant.
Once submitted, this application will not be considered complete until your supervisor provides a recommendation statement and acknowledgement via the approval email that will be generated by the system.
Because of this, please make sure you discuss this with your supervisor prior to submitting your application. This way, they will understand the approval request when they receive it in their email.
