* The following agreement is between Pattern Tester (Applicant's Name) and Chalia Delarosa of “Chalia’s Craft” effective the date of the submission of application (Today's Date) and deadline.

* The Pattern Tester Questionnaire, and two 1:1 photos must be submitted by the deadline specified by Chalia Delarosa.

* Unauthorized reproduction, distribution, or sale of this pattern, or images in part or in whole are prohibited.

* I the applicant agree to purchase the published pattern, the day it is released, upon failure to complete the pattern test by the appointed deadline.

* I the applicant grant Chalia's Craft permission to use my photos from the test for reproduction, distribution, and marketing purposes.

* I the applicant agree to be responsible for obtaining all materials necessary for the completion of the pattern test.

*The term of this agreement starts upon the submission of the application and ends when the below completed criteria have been met to the satisfaction of “Chalia’s Craft."

1. Completion of the Pattern Tester Questionnaire.
2. Two 1:1 photos minimum of your finished work. The photo must be taken in a well lit environment, with a clean background.

*The application will be reviewed by "Chalia's Craft" within 48 hours of submission. Upon review and final acceptance,  the applicant will receive an email containing the PDF Crochet Pattern document, and a link to the Pattern Tester Questionnaire.
