This registration is required for a Covid-19 test. Due to the drive-thru nature of the testing, we can only allow up to 4 members of a household per vehicle per booking. PLEASE NOTE: Public testing is for residents of the two towns or employees/students of HWRSD who are symptomatic, or have been designated as a close contact of a positive or probable case, or have been directed to test by a Public Health Nurse or a member of the Contact Tracing Collaborative. If you are seeking testing due to travel, peace of mind, or informational purposes, check the appropriate box so that community members with symptoms or exposure can access the public testing they need. Close contact is currently defined by MA DPH as a person within 6 feet of an infected individual, for at least 10-15 minutes, while they were symptomatic or within the 48 hours before symptom onset.

Finally, those that are seeking private testing, you can use this form to sign up as many as 4 members of your household to get tested at MRMS during the public testing at a cost of $85 per test.
