Field Trip Permission Form 2019-2020

Please be aware that you will still receive notification and information each time your child leaves school on a field trip with the exception of Walking Field Trips, therefore if you do not give permission for your child to participate on a specific field trip there will be time to make other arrangements.

Field Trip Permissions

YES, my child has permission to attend supervised school related field trips during the school year. We understand the arrangements and believe the necessary precautions and plans for the care and supervision of the students during the trip will be taken. Beyond this, we will not hold the school or those supervising the trip responsible.

YES, my child has permission to participate in walking field trips in the Spero Academy neighborhood (four block radius) throughout the school year. I believe that reasonable precautions and supervision will be provided for these walking trips.

Field Trip Medications

The administration of medication to students on field trips is done only when the student has a medical condition that may be adversely affected without medication. This applies to both prescription and nonprescription medication. The student’s teacher will be responsible for storing and administering medication on the field trip.
