This application form is due on December 11th at midnight, but please begin sooner as you may have to collect some information to complete the application. Accepted participants will be notified 1-2 weeks after the submission deadline.

The Health Profession Recruitment and Exposure Program - "HPREP" - is an outreach program designed to provide high school students with the opportunity to interact with medical students, faculty and health professionals. HPREP is sponsored by the Duke University School of Medicine Chapter of the Student National Medical Association (SNMA). We generally accept applications from 9th- 11th grade high school students from across the Triangle and central NC area who are motivated and interested in medicine, allied health careers or science.  This year we WILL NOT be accepting applications from City of Medicine Academy 9th Graders. We also WILL NOT be accepting applications from any 12th grade students due to a program shift, but will attempt to host virtual office hours on a monthly basis (You can complete the first section of this application to denote your interest). HPREP is designed for members of underrepresented minority or ethnic groups, although others are welcome to apply. According to the American Medical Association, the term “Underrepresented minority” is defined as "those racial and ethnic populations that are underrepresented in the medical profession relative to their numbers in the general population." This includes traditionally defined underrepresented minority groups, African-Americans, Mexican-Americans, Native Americans (American Indians, Alaska Natives, and Native Hawaiians), and mainland Puerto Ricans, in addition to other minority groups.

Tentative Dates for HPREP sessions are January 28; February 11 and 25; March 11 and 25; April 8 and 22; and May . If you know you will miss 2 or more of these dates you should not apply.

Time: Sessions typically occur from 8:45am to noon

Location: Most sessions will be IN PERSON this year with very few zoom session. In person session(s) will be held in a building adjacent to Duke University Hospital (2301 Erwin Rd, Durham, NC 27710). If you feel transportation may be an issue still apply, we are developing workarounds to potential transportation issues.

2) Unofficial transcript or grades from your school's online portal - screenshot(s) or picture(s)
