Well-being Fair - Presenter Form
Thank you for being interested in the Well-being Fairs at Mukunda Studio. On December 18th from 1-5pm, we will host our first, seasonal well-being fair to highlight 8-10 local area wellness leaders who share services, practices and strategies for well-being. 

Please fill out this form so that we may have a better idea of the service you'd like to present/share. We are primarily seeking practitioners, educators and specialists who can provide information on services versus a fair that is filled with products and material goods to sell. 

Our goal is to help our guests learn of various ways that well-being can be generated from lifestyle practices/choices, community involvement, connection with self and nature, etc. which feel accessible and simple. We are open to many ideas and interpretations for this first event!

We host monthly community discussions called, EmpowerHour. Would you like to be a part of our community discussion on well-being from 1-2:15pm? (This is live and live-streamed and will occur during the opening period of the event. This is an interactive discussion and will require 10-15 minutes of your time away from your table to share about your offering.) *
We have a freight elevatory for carrying large or heavy items from the ground level to the 2nd floor studio. Will you need to utilize this or will the flight of stairs from ground level to the 2nd floor studio be accessible? *

Give any details for how you imagine presenting your service or information at the fair. You will have approximately a 6'x6' area in a room that will be set up for 8 presenters - 4 on each side of the room, side by side. You can have a table, chairs, etc. *
