Your child's information will remain absolutely confidential and will only be used by Aathi Yoga Association and its Kids Yoga Camp co-ordinators.  The more information you can provide the better we can meet the needs of your child and take appropriate action in case of any health emergency.

For the purpose of this form, the abbreviation AYA is used to refer to Aathi Yoga Association, who is conducting the MARCH BREAK KIDS CAMP PROGRAM.  AYA would like to request your permission to take photographs and record audios and/or videos of your child participating in our MARCH BREAK KIDS CAMP PROGRAM.  The photographs and audios/videos taken during the program will be used by AYA solely for its own informational, promotional, advertising, and/or communication purposes.  By signing this form, you are consenting to the taking of photographs, audio, and/or video recordings of your child by AYA for the purposes mentioned in the above paragraph, you are assigning to AYA, and waiving any rights you have related to, any such photographs and/or video recordings, and you are consenting to the use of any such photographs and/or video recordings, in whole or in part, by AYA.  

"I understand and agree that I will receive no monetary compensation for the use of the photographs, video, and/or audio recordings of my child.  Therefore, I hereby agree to allow photographs and/or video recordings to be taken of my child, while he/she is participating in the MARCH BREAK KIDS CAMP PROGRAM, to be reproduced, published, displayed, broadcast, transmitted, licensed, or otherwise used by AYA or in connection with the purposes described in the first paragraph, including without limitation on AYA's internet web sites, in AYA's printed materials, or in any other materials or medium whatsoever and wherever.  I understand that the photographs, audio/video records are being created under the direction and control of AYA. I hereby irrevocably assign to AYA and all rights, including copyright, financial or other rights, and I hereby irrevocably waive in favour of AYA any and all moral rights or rights of similar nature that I may have of the photographs, videos and/or audio recordings.  I agree that AYA has the sole ownership and rights of the materials produced, including copyright interests, and I acknowledge that I have no interest or ownership in any materials produced or its copyright. I agree that I will not bring or consent to others bringing a claim or action against AYA on the grounds that anything contained in any materials produced by AYA, or in the manner in which those materials are used, is defamatory, reflects adversely on me, or violates any other right whatsoever, including, rights of privacy and publicity. I hereby release and forever discharge AYA and their respective members, directors, agents, partners and affiliates, personal legal representatives, and successors, from all actions, claims, causes of action, suits, demands, liabilities and damages whatsoever, in law or equity, which I may have against any of them in connection with the materials produced by AYA using the photographs, and videos/audios taken during the MARCH BREAK KIDS CAMP program.  I understand and agree that my child is not allowed to bring any type of cameras (cell phone included) and not allowed to take pictures of the program activities. By signing my name, I (and my legal guardian, where applicable) acknowledge that I (or we) have carefully read and understood this form and signing it out of our own will."

I agree to the following:  

By allowing my child to participate in the MARCH BREAKD KIDS CAMP PROGRAM (consisting of Yoga sessions, fun games, dancing, interactive workshop on healthy digestive system), I understand that some of the activities involve physical exertion and I am fully aware of the risks involved.  Therefore, I hereby waive any and all claims, and agree not to hold The Aathi Yoga Association, its committee, its members, its board of directors, the March break kids camp directors/coordinators/leads/volunteers/teachers, and the venue (where the program will be conducted) the Parkland Public School located at 18 Coxworth Ave, Markham, ON  L3S 3B8 responsible for any injuries, including and special or consequential damages, suffered by my child or any loss that is caused in whole or in part by my child's failure to follow instructions correctly and any accident, injury or loss that occurs during the whole period that my child is participating.  I understand that I am registering my child to participate in the program with the understanding of any inherent risks. Further, I guarantee that my child is fit and have no medical conditions that would put them in danger while participating.  I voluntarily, knowingly, waive any claim that I may have against the Aathi Yoga Association, its committee, its members, its board of directors, the March break kids camp directors/coordinators/leads/volunteers/teachers, as well as the Parkland Public School located at 18 Coxworth Ave, Markham, ON  L3S 3B8 where the program will be conducted.
                                                                                                                                                                                                      I certify that the answers and information I have provided in this Registration and Waiver Form are true and complete to the best of my knowledge and that the Aathi Yoga Association (AYA), its volunteer members including the executives, board of directors, and the volunteers who are conducting the program, or other organizations associated/affiliated with The Aathi Yoga Association will not be liable for any damages or injuries or illness or conditions or any other mishaps arising from my child's participation in the activities.
