COVID-19 Test To Stay Consent Form/Formulario de consentimiento para la prueba COVID-19
As the guidance has changed from the Department of Health, we are able to offer your child a “Test To Stay” initiative to assist keeping them in school during a “close contact” exposure to COVID-19.  This initiative is available for Unvaccinated Students only.

Students who are unvaccinated who came into close contact with someone at school with COVID-19 can participate in the Test To Stay program.  This would allow your child to remain in school and ride the bus to school.  They still need to quarantine outside of school activities for at least 5 days.

These tests are available to those who are close contact due to school exposure only.  

If you wish to have your child participate in this initiative, please complete the consent form below and either accompany your child to school or send it in with your child.  The child should provide the consent to the school nurse.

The test to be administered is a Rapid Test.  It takes no longer than 15 minutes to receive the results.  A sample of the lower nostrils is done with a cotton swab.  It is then put into a testing solution card for 15 minutes.  Results are either positive or negative.  If positive, you will need to take your child home and contact your healthcare provider. If negative, your child will remain in school.  This test would be completed on 3 out of 7 school days.
