I do hereby release Youth With A Mission, Inc., its agents, employees, and volunteer assistants from any liability whatsoever arising out of any injury, theft, damage, disability or loss or health, property, emotional stability or life, which may be sustained by myself or other persons during my/their course of involvements with Youth With A Mission.

If applicant is under 18 years of age than the signature of parent / guardian is also required.

I hereby declare that:
a) As an adult I have never been involved in, or convicted of violent, drug related, sexual assault or any other crime against a minor.
b) I understand that checks may be carried out to verify the above and hereby give my permission for any police checks which might be necessary.
c) I will provide police checks from my home country & the countries I've lived in the last 5 years for more than 1 year.

If applicant is under 18 years of age than the signature of parent / guardian is also required.

In the event of an emergency in which I am rendered unconscious and my nearest responsible relative or guardian cannot be contacted, I hereby agree to such treatment, anaesthetics, transfusions and operations to be performed upon myself as in the opinion of the attending physician/s is deemed necessary.
I declare that I shall not hold Youth With A Mission, Youth With A Mission Thessaloniki, the base director or his/her delegate, liable for any decision made by him/her for any damage or loss that I sustain as a result of exercising the authority herein granted by me.

If applicant is under 18 years of age than the signature of parent / guardian is also required.

In case of my death during the course of my involvement with Youth With A Mission, I wish that my next of kin be advised as soon as practical and that their wishes with regard to funeral and disposal of my body be complied with so long as my next of kin places Youth With A Mission in sufficient funds to carry out those wishes. In the case where Youth With A Mission is unable to contact my next of kin or that my next of kin are unable or unwilling to give directions at to funeral and disposal of my body and come to a satisfactory arrangement with regard to payment of related costs within reasonable time, then I direct that Youth With A Mission at its sole discretion make arrangements for funeral and disposal (including burial in a foreign country) at the expense of my estate.

I confirm that I understand payment of the required school tuition and fees must be made on or before my arrival, unless otherwise arranged with leadership, and I agree to do so.
I also confirm that I am fully aware of my financial obligations, both to the Lord and to the trainees and staff at the school.
I therefore accept all responsibility for my fees, tuition and personal expenses incurred during my involvement with Youth With A Mission.  
I understand and agree to the following refund procedure should I leave the lecture phase early for any reason.
1st or 2nd week 60%, 3rd or 4th week 50%, 5th or 6th week 20%, 7th week and on 0%.
In case of needing to leave the Outreach Phase early, I am responsible to cover the extra costs for flight changes, transportation, accommodation and food.

I understand and agree to abide by the following guidelines for the duration of this training program. I understand that if I do not do so, I may be asked to leave this program and if applicable have my visa revoked.
– I understand due to the multicultural and interdenominational nature of Youth With A Mission that the consumption of Alcohol, Drugs & Tobacco are not allowed.
– I will not purchase, sell, have on my person, bring onto YWAM Thessaloniki property, distribute or use any illegal substances.
–I will not commit any sexual immoral acts.
