Neurodivergent - N50,000/week
Neurotypical - N40,000/week

FOS CREATIVE STUDIO MEDIA/IMAGE RELEASE FORM

In the course of your child's time at the FOS Creative Hub, your child will be recorded, filmed and photographed in order to document a variety of learning experiences. This material may be used but is not limited to our library, media center, print media, website, social media websites and so on.

In this consent form we are asking for your permission to take, store and use images or videos of your child for further purposes related to the FOS Creative Hub, these will include promotional activity and publicity through printed material, our website and social media/ networking services.

Please be assured that real names will not be used and images will be carefully and sensitively chosen and will not be used out of context.
Thank you.

Your consent authorizes FOS Creative Hub to use your child's photograph, and/or audio/video/film production for publication related to the FOS Creative Hub functions and activities. Once submitted, this form will remain in effect for the duration of your child's stay at the FOS Creative Hub unless otherwise revoked by notifying the management in writing. *
