This year, as you surely know, is very different. For that reason, please read and answer each item carefully with the most up-to-date information possible. Due to the amount of information we hope to gather from you, this survey is lengthy. Google Forms (which is what this survey uses) does NOT allow someone to start a survey and save their progress if they can not continue for some reason, so it is important that you give yourself enough time (10-15 minutes) to fill it out and click 'Submit' at the very end. Again, without clicking 'Submit', all previous answers will be lost.
