Please fill this form out ONCE for each unique 4-H member. (Do not include multiple family members together, or your priority for projects will be dropped to 5th priority for all family members.)

Then, use this form to number the projects you wish to take in order of priority. (Project #1 would be the project you most want to take). If this is your first year in 4-H, consider only taking one or two projects!
Project rosters will be compiled based on a member’s stated priority.

PLEASE NOTE: Some projects have limited enrollment, due to space constraints. Waiting lists will be compiled for those projects that exceed the maximum size.
Members signing up for projects after October will be subject to project availability.
Some projects (like Rising Stars, State Exchange and Square Dancing projects) are countywide projects. It is not necessary to sign up for those through this Bayside 4-H Project Sign-Up Form.
Any project fees are paid directly to project leaders at the first project session.

Please select the projects you are interested in taking this year, by priority, starting from the one that you MOST want to take. Only one project may be selected for each priority. For projects marked as LIMITED ENROLLMENT, you may be placed on a waitlist. The project leader will let you know if there is any space available.

PLEASE NOTE: NOT ALL PROJECTS WILL BE OFFERED THIS YEAR. Some leaders are trying to figure out how much interest there is in each topic and will adjust which projects are offered based on the interest shown for this year.
