Forest Bathing | Health Awareness
Greetings! I am looking forward to guiding you on the upcoming forest bathing walk!

A forest bath is very slow-going, low-impact, short-distance stroll—sometimes less than .5 mile, sometimes up to a mile or two. The terrain can vary and be uneven, slippery or icy in certain weather and seasons, sometimes with hills to climb, and wet and mushy in spring. We will walk rain or shine. Please use your own best judgement on whether or not this walk is safe and right for you. Unless indicated, walks are not considered accessible, as most are trails winding through woods and field. Please note that you must be able to amble of your own accord. Shinrin-Yoku Madison and lead guide Kate Bast are unable to offer assistance or equipment to help you do so.

This brief, confidential survey simply helps me have a very basic awareness of any general health concerns that may impact you on the walk. Please use your own best judgement about participating.

Thank you!!

Do you have any physical issues that may make it difficult for you to walk on narrow, uneven, unpaved trails? If so, please explain briefly, below. Please bring an assistive device if need be.
Do you have any heart health issues or concerns, especially any that arise during physical activity? Please explain briefly.
Do you have any allergies—food, seasonal or otherwise? If yes, please indicate specific allergies and reactions to allergens. I serve a light refreshment, and this information is valuable to helping me choose and provide, to the best of my ability, safe options.
Do you carry an Epi-pen or other fast-acting medication that the group should be aware of?
Clear selection
Please note any fast-acting medications, including Epi-pen or other epinephrine injector, that you will carry on the walk. Indicate it's purpose and where you typically keep it on your person (I will reconfirm that day, as well, so the group is aware should they need to help you access your injector or medication so you can administer it.)
Any other concerns or health issues you feel I should be aware of for our forest bathing walk?
A copy of your responses will be emailed to the address you provided.
