Please complete a separate form for each student in grades K-5.  We are excited to see you this fall! You will need your child's Dominion Trail School Bus Information to complete this form. You can find their bus information in your ParentVue account.  It is important that you tell us YOUR PLANS for how your child will be getting home from school. These plans will be shared with your child's teacher so they can safely release them from school.
Please do not use bus information from previous years, this is a new year with new routes, new buses and new schedules. You will want to know this information as well for your child's safety.

In the event of early closing of school due to weather or other causes, each student must have specific instructions for getting home or to the daycare provider. School personnel cannot call parents nor should parents call the school as the phone lines need to be kept open for emergencies and instructions from the central office. Please indicate below what your child should do in the event of an early dismissal. *
