By submitting this form, I hereby consent to my child's participation in the Maranatha Evangelistic Ministries VBS on July 14, 15 and 16, 2022 at 28000 New Market Road, Farmington Hills, MI 48333. I understand that this event will take place in and around the church grounds and that my child will be under the supervision of authorized church personnel. I acknowledge and accept full responsibility for safety, liability, and medical insurance for my children during the event. In case of any emergency, I will not hold any of the event committees and other officials of the Church responsible. Any damage to the Church and its premises caused by me or by the children registered under me will be my responsibility. I understand that by participating in this event, I consent to the photo images and videos taken by the event media team during the VBS and recreation activities for the event. I understand that I need to contact Jojy Kuriakose If I want my children’s pictures or videos excluded from the church’s website or social media. I also consent not to share photos of the event in social media or other websites without prior permission from the event media team.
