RMR Requires trainers to carry $1Million liability policy naming Rein Maker Ranch as "named insured".  Please list your policy number, insurance company name below and provide us with a copy of the liability cert naming RMR. *
Are you authorized by your clients to provide equine care to their horses including but not limited to: Farrier services, veterinary care, transportation, training, feed program, supplement program and general care of the horse(s) owned by your clients? *
Will you be collecting board payments from your clients and paying RMR, or will the client be responsible for paying RMR directly? *
Your Veterinarian's Contact Information.  Please provide Vet's name and phone number so we can make best efforts to contact your vet in the event of an equine emergency.  If we are unable to reach you or your veterinarian, and we are made aware of an emergency we will contact the RMR vet on call to respond. *
