By writing your name below: 1) You give permission for your child to attend the Rockaway Valley School Ski Club trips on  Wednesdays, January 8, 15, 22, 29,  and February 5, 2025. 
2) You agree that all dates are tentative and dependent upon the weather.  3) You understand that the ski club will take a total of 5 ski trips and that in the event of cancellation due to poor weather conditions, this permission form will grant approval for your child to participate in any rescheduled trip. 4) You understand there are no refunds of any kind after the registration period ends.

By writing your name below, you agree to indemnify and hold harmless the Boonton Township Board of Education and its agents, servants, members, club advisors/chaperones, and employees from any liability and claims for damages, losses, costs, or charges stemming from ski club activities. *
I understand that by submitting this form I acknowledge student participation in the RVS Ski Program and understand the rules and procedures of the club. *
Remember: A check for $310.00 made out to "Boonton Township Board of Education" is required to secure your spot in the ski club. Information to register with Shawnee will be sent to you once your check has been received. Feel free to contact me if you have any questions: csnider@btrvs.org. *
