Please express your interest in serving on the Board of Directors for the 2025 season of South Fork Panthers of Winston Salem, Inc. by completing the form no later than October 15, 2024.

By completing this online form, I am nominating either myself or the individual named below to be considered by the 2024 Nominating Committee for a position on the Board of Directors.

It is important to note that serving on the Board requires attendance at meetings, involvement in policy-making, and support for fundraising activities approved by the Board. This includes supporting the organization as a whole.

Additionally, all Board members are expected to adhere to the following Board Covenant:
- Participate in organization-wide fundraisers
- Serve on at least one Board committee
- Volunteer during both home and away games
- Attend no more than three Board meetings per year
