Please complete the Band/Chorus/Strings registration for the 2019-2020 school year. The ensemble choice that is selected on this form will be taught consistently as a yearlong course for the rest of your child’s fifth grade year.

Please complete the information below to indicate the Ensemble preference for your child by WEDNESDAY, MAY 22, 2019. The Lower School may select an ensemble for those who do not return this form.

You will be contacted at a later date by the teacher of the ensemble your child has chosen with information about the class.
