Participant information:
- Each participant will need to fill out a form (family members will need to fill out multiple forms).
- All new students MUST attend at least 3 of 4 sessions.
- Open to adults, teens ages 14+ (or will be turning 14 soon)
- The cost is $40 to participate in the training course,
- Payable upon arrival by cash/check or prepay via VENMO at the end of this form.
