If your student is enrolled for ASCEND Part-time Care (3 days-per week) or Drop-In Care (1 to 2 days per week) please use this form to communicate the dates your student is to be collected for ASCEND in the afternoon for the upcoming week.  We ask that you also communicate these dates to your child's teacher.  This form should be entered for each student attending.  *This form is to be completed AFTER Registering your student in MSBActivities.

This form must be submitted by the end of day Friday of the week before the dates you are selecting.  Day or week-of registrations are not able to be accommodated.
