Please fill out this form if you wish for your student to take home a device for online learning, in the possibility that schools are closed next week. If you have more than one student in your family that needs a device, you must note in the form how many students need devices.

Keep in mind it is the responsibility of the parent and student to make sure the device is taken care of properly and brought back to school on the next school day. If the student forgets the device at home, a family member must bring it in the immediately upon notification in the morning of the day school resumes.
