This registration form takes approximately 3-5 minutes for returning families and 5-10 minutes for new families. If you have any questions contact the school office at 847-438-7921.

Registration Fee: Completion of this form before 8:00 AM on Tuesday, February 4th reserves your student's seat and the registration fee is waived for these early registrations. After 8:00 AM on February 4th, families should still complete this form, but registration is not complete and the seat is not reserved until the school office has received the $150 family registration fee. You may stop by the school during office hours (7:30am-3pm M-F) or mail a check for $150 made payable to St. Francis de Sales School.
