Welcome to the Fanshawe Password Reset Portal Feedback Form. The feedback form will run from Monday, June 17, 2019 until Friday, August 2nd, 2019.

By completing this form and fulfilling the requirements below, you are agreeing to be entered into a draw to win 1 (One) of 2 (Two) Beats PowerBeats3 Wireless headphones.

Contest Entry Rules

This contest is open to all Fanshawe Students, Staff and Applicants.

All contestants must fulfill the requirements below:

1. Contestants must have accessed or attempted to access the Password Reset Portal (https://password.fanshawec.ca)
2. Feedback provided in this form must be related to your experience in using the password reset portal
3. Feedback must be detailed (i.e. not single word answers - please explain where required)
4. Agree to have the Fanshawe IT Service Desk be able to follow up on the feedback provided

Your student or employee number below is required to be entered into the draw. Winners will be picked on August 2nd, 2019. The IT Service Desk will contact the winners using the information we have on record associated with the student/employee number.
