VBS 2020 Rocky Railway Registration
Welcome! This online form is for the parents of any child/children in Kindergarten through 5th grade who are interested in registering as participants for Summer VBS Rocky Railway at Prince of Peace Catholic Church which will take place August 10-14th, 2020 from 9 AM-12:45 PM.

Due to restrictions on social gatherings and CDC recommendations related to COVID-19 and for the safety of everyone involved we are required to follow and enforce the following rules strictly:

1. Social Distancing- The children must be able to follow social distancing rules which means staying 6 ft. apart from other children, volunteers, staff, and others throughout the event. No high fives, hugs, handshakes, etc.
2. Masks are required for each child when indoors and will be used only when going indoors (such as for mass and the bathroom or if the weather requires it). Masks are optional when outdoors and are not recommended while playing games and sports. Every child should bring a small bag or book bag daily where they can keep their mask. Children who have respiratory problems and can't wear a mask will be welcomed at the parent's discretion. A facial shield will be provided for those children who can't wear a mask while indoors.
3. Daily Contactless Temperature Checks- Children will be required to have their temperature checked before joining their small group for VBS. Anyone with a temperature over 100.4 degress Farenheit will not be allowed to participate.
4. Before participating, each participant must submit a signed Diocese of Orlando Assumption of Risk and Waiver of Liability Relating to Coronavirus/COVID-19. Forms will be provided to you by e-mail after registration.
5. Beginning with August 10th and every subsequent day of VBS, parents will be asked a series of questions which they must be able to honestly answer no to regarding their child. This questions are the following:

In the last 14 days, have you or a family member experienced a new cough?
In the last 14 days, have you or a family member suffered from a shortness of breath?
In the last 14 days, have you or a family member suffered from body aches that you cannot attribute to a specific activity such as physical exercise?
In the last 14 days, have you or a family member experienced a sore throat?
In the last 14 days, have you or a family member experienced a loss of the sense of taste or smell?
In the last 14 days, have you or a family member experienced a fever over 100.4 degrees Fahrenheit?
In the last 14 days, have you or a family member been exposed to someone who has a confirmed diagnosis of COVID-19 or is awaiting results?

For our daily Children's Worship we will have a Catholic Mass. Is your child Catholic and is he/she able to receive Holy Communion which means they have already had their First Communion and regularly attend Sunday Mass? If you have more than one child participating, please tell us which ones can receive Holy Communion. *
Any Special Requests? Is there a friend you want to be grouped together with? Do you need to have someone other than a parent/guardian drop off or pick up the child? ( This year, we will be grouping children of the same ages together in order to maintain social distancing. We will do our best to honor your request.)
Are you experiencing financial hardship due to loss of income/employment due to COVID-19? If so, please let us know and we would gladly waive the registration costs for your family?
