This Document will be used to track all of the teams registered for the virtual 2021 S/SW Intercollegiate Medical Device Make-a-Thon. Please have every member fill this form individually.

NOTE: CAD must be done on SolidWorks. Be sure at least one person on your team is familiar and has access to SolidWorks during the competition.
