NOTICE OF COLLECTION  Personal information on this form is collected under the authority of sections 58.5(1), 169.1-173 of Education Act, R.S.O. 1990, c.E.2 (“Education Act”) (as amended), subsection 11(3) and section 20 of the Ontario Regulation 298 under Education Act and will be used by Toronto District School Board (“TDSB”) to support student learning, execute TDSB’s functions/programs and administer the relationship between TDSB and its students, schools, staff and others. The information is retained in accordance with Municipal Freedom of Information and Protection of Privacy Act, R.S.O. 1990, c.
