5. Voucher Program
a. Students from Public School will not submit voucher certificate
b. Qualified Voucher Recipient Certificate (to follow from private school students only)
c. ESC Certificate (if grantee & from private school students only)
6. 2 Long Colored Folders (color varies depending on Strand)
a. STEM - Green
b. ABM - Yellow
c. HUMSS - Blue
d. ICT - Orange
e. H.E. - Pink
f. SPORTS - Red
7. Transcript of Records (SF-10) *to be requested from previous school once officially enrolled
