This form is only for those that are registered in Ventura AYSO - Region 39. If you are registered in any other Region you must contact that Region regarding their refund policy.
This form must be completed and submitted in order for Ventura AYSO - Region 39 to process a cancellation of your child's registration from the current soccer season and generate a refund.
*** All refunds are subject to eligibility under the Region 39 Refund Policy, which may be viewed at:

* The $20 AYSO National Membership Fee is non-refundable under any circumstance.
* All transaction fees charged for registration are non-refundable.

* GENERAL REFUND - Form must be submitted by July 15, 11:59 PM
Refund requests submitted prior to August 1 will be entitled to a full refund less the $20.00 AYSO National Membership Fee, $15 Processing Fee and any transaction fees.
* 50% Refund - Form must be submitted July 16 - July 31, 11:59 PM
