I acknowledge that I must carry proof of age and eligibility with me at all times and be ready to produce this proof if asked by the referees or tournament officials and/or staff. Proof of age and eligibility consists of laminated player cards with pictures issued by either USYSA or US Club Soccer (virtual cards with pictures will also be accepted). If you do not have player cards, you will need to provide proof of registration in the form of a letter from your club registrar certifying that your player is indeed registered and eligible for the 2020/2021 seasonal year. The letter should be on club stationery and include team name, player name, player date of birth, and player ID number. This letter must include all players for which you do not have player cards. Failure to provide proof of age and eligibility will result in a forfeit. *

I acknowledge that all players must be listed on my roster in Got Soccer, including guest players, and that this will serve as my official roster. All players must be added by August 28, 2020. I acknowledge that no write-ins will be allowed. (Please make sure you print a game card to verify that all players are listed.) *

I ACKNOWLEDGE THAT ALL SPECTATORS MUST WEAR A MASK AT ALL TIMES. If a spectator is not wearing a mask, the referee will let the coach know and it is the responsibility of the coach to remedy the situation. If it is not remedied or continues to happen, the referees will abandon the game and the win will go to the team that is in compliance.
